Application Process
If you are ready to apply our online application can be found below. You can also give us a call at 612-676-3700 to schedule a time to fill out the application in person at one of our office locations or request a paper copy be mailed to you directly.
Once we receive your application, you will also be required to submitted state required identification documents. These requirements are outlined below, but if you have any questions on this process please don't hesitate to reach out.
If you have submitted your application and all required documents, we are working on processing your enrollment into the program. Please keep an eye out on your email for an electronic signature request. This is to ensure that all of your information has been entered correctly.
Once this is signed, somebody from our team will reach out to you to schedule your Individual Employment Plan Meeting!
